Open Doors to Therapeutic and Consulting Services, LLC maintains a fee schedule identifying the price of various services. If you have insurance and are receiving service covered by your insurance, the fee schedule may not accurately reflect our agreement with your insurer, which may affect the amount you are responsible for.

Self-Pay Rates: 

Initial Evaluation/Assessments: $200.00

Individual Therapy 45 minutes: $130.00

Individual Therapy 60 minutes: $150.00

Family/Couples Session 45 minutes: $160.00

Family/Couples Session 60 minutes: $180.00

Group Counseling 60 minutes: $50

Clinical Supervision per hour: $100

Insurance

We accept Aetna, Anthem, Cigna, Optum, EAP, and Self-pay clients.

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• Is approval required from my primary care physician?

 

Co-payments

All applicable co-payments, deductibles, or any other out-of-pocket expenses are expected to be paid at the time of the appointment. The co-payment is the patient’s responsibility and payments are expected at the time of appointment unless the patient’s insurance coverage requires another arrangement.

 

Method of Payment

Payment is accepted by all major credit cards. Open Doors to Redefining Therapeutic and Consulting Services, LLC has a credit card policy where all clients are required to leave a credit/debit card on file to alleviate balances. A credit card authorization form will be completed at the time of your intake appointment and your therapist will review this policy with you should you have any questions.

 

Reservation Rights

Open Doors to Redefining Therapeutic and Consulting Services, LLC reserves the right to increase fees in the future to a reasonable amount, upon reasonable advanced notice.

 

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a medical plan or have coverage or eligible for a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

• You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

• Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your psychological service. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 business hours (Monday-Friday) beforehand. Otherwise, you will be charged a $99 no show fee. In addition, if you need to reschedule your appointment, please  make sure you reschedule at least 24 business hours (Monday-Friday). Otherwise, you will be charged a $99 late cancellation/reschedule fee.
